We undertook a cross-sectional study in 2020, focusing on census tracts and aggregated dissemination areas (rural Canadian application) within 14 U.S. and 3 Canadian jurisdictions. Our research did not incorporate census tracts or areas having a population density of less than one person per square kilometer. Information derived from a 2020 audit concerning timely medication access was used to locate clinics that enroll new patients within 48 hours. The influence of population density and sociodemographic factors on three different outcome measures was analyzed employing unadjusted and adjusted linear regression models. These outcomes were: 1) driving distance to the nearest methadone clinic accepting new patients, 2) driving distance to the nearest methadone clinic accepting new patients for medication initiation within 48 hours, and 3) the difference in the two driving distances.
To further our investigation, we considered 17,611 census tracts and areas with a population density exceeding one person per square kilometer. After considering regional differences, US jurisdictions were found to be, on average, 116 miles (p-value < 0.0001) further from a methadone clinic accepting new patients and 251 miles (p-value < 0.0001) further from a clinic accepting new patients within 48 hours, when compared to Canadian jurisdictions.

Employing linguistic guidelines promulgated by the federal National Institute on Drug Abuse (NIDA), we investigated the evolving use of pejorative terms associated with addiction within four prominent public communication channels: news articles, blogs, Twitter feeds, and Reddit forums. Over a five-year period (2017-2021), we analyze percent changes in article/post rates employing stigmatizing terms by fitting a linear trendline. Statistical significance of trends is assessed via the Mann-Kendall test.
Over the last five years, a substantial decline in the use of stigmatizing language was seen in both news articles (682% decrease, p<0.0001) and blogs (336% decrease, p<0.0001). A study of social media content indicated a rise in stigmatizing language usage on Twitter (435%, p=0.001), in contrast to a stable occurrence on Reddit (31%, p=0.029). Pulmonary hypertension (PH), defined by irreversible pulmonary vascular remodeling (PVR), is a disease that progresses to right ventricular failure and ultimately ends in death. Early macrophage activation is demonstrably essential for the progression of both PVR and PH, but the intricate molecular mechanisms responsible are still obscure. Earlier work highlighted the role of N6-methyladenosine (m6A) modifications of RNA in driving the phenotypic transformation of pulmonary artery smooth muscle cells and their connection to pulmonary hypertension. We demonstrate in this study that Ythdf2, an m6A reader, plays a pivotal role in regulating pulmonary inflammation and redox balance in PH. In a mouse model of PH, a rise in Ythdf2 protein expression was noticeable in alveolar macrophages (AMs) during the early stages of hypoxia. Mice lacking Ythdf2 specifically in myeloid cells (Ythdf2Lyz2 Cre) experienced protection against PH, marked by reduced right ventricular hypertrophy and pulmonary vascular resistance, in contrast to control mice. This was associated with a decrease in macrophage polarization and oxidative stress levels. The absence of Ythdf2 correlated with a considerable increase in the expression levels of heme oxygenase 1 (Hmox1) mRNA and protein in hypoxic alveolar macrophages. Ythdf2, mechanistically, promoted the degradation of Hmox1 mRNA in a manner dependent on m6A. Moreover, a hindrance of Hmox1 resulted in macrophage alternative activation, and reversed the hypoxia protection evident in Ythdf2Lyz2 Cre mice under hypoxic conditions. Through our analysis of combined data, a novel mechanism connecting m6A RNA modification with alterations in macrophage function, inflammation, and oxidative stress in PH was observed. This study identifies Hmox1 as a downstream target of Ythdf2, potentially making Ythdf2 a therapeutic target in PH.

In elderly patients with untreated acute myeloid leukemia (AML), diagnosed according to WHO guidelines, we compared the clinical efficacy of azacitidine (AZA) and decitabine (DEC). Of the patients studied, 139 were in the AZA group and 186 in the DEC group.
